Sickness Certificates (Fit Notes)

Home Visits

House visits are only available for patients who are housebound because of illness or disability.

If you need a home visit, you can help us by calling reception before 10am.

Telephone: 01322 427447

A doctor, paramedic or nurse may phone you back as it may be that your problem can be dealt with by telephone advice, or that it would be more appropriate to send a paramedic or nurse, or indeed arrange a hospital visit.

Please remember that several patients can be seen in the practice in the time that it takes to make one home visit. There are also better facilities for examining and treating patients at the Health Centre.

Change or Cancel an Appointment

Please give us as much notice as possible so we can offer your appointment to someone else.

Ways to cancel

1. Telephone us on 01322 422 775 and tell us your name, date of birth and date and time of appointment.

2. Cancel using our online form

How to change and rebook an appointment
